As we mark Sarcoma Awareness Month this July, it is time to spotlight a silent warning sign that many people ignore: painless swelling.

While it is natural to think “if it doesn’t hurt, it can’t be serious,” this assumption can be dangerous. Dr. Choong Chee Leong, Consultant Orthopaedic Oncologist, warns that a painless lump or swelling could be a sign of sarcoma, a rare but potentially aggressive cancer affecting the soft tissues and bones.

Why Painless Swelling Should Never Be Ignored

One of the most common misconceptions Dr. Choong encounters is the belief that pain is the only indicator of a serious problem. Many sarcomas begin as painless lumps, especially in the limbs or trunk. That lack of discomfort is exactly why diagnosis is often delayed.

“Any new or unusual growth should be investigated promptly,” says Dr. Choong. “Even if it’s painless, it could be something serious.”

In addition to swelling, other red flags may include:

  • Reduced mobility
  • Unexplained fatigue
  • Sudden weight loss

If any of these symptoms accompany a lump, it is essential to seek medical advice without delay.

The Importance of Early Diagnosis

Catching sarcoma early can dramatically improve treatment outcomes. A thorough evaluation typically involves:

  • Imaging (MRI or CT scans)
  • Biopsy to determine if the mass is malignant

If diagnosed, a multidisciplinary care team will often recommend a combination of surgery, chemotherapy, and/or radiotherapy depending on the stage and type of sarcoma.

“Early diagnosis gives us more options,” explains Dr. Choong. “The earlier we intervene, the better the outcome for the patient.”

Limb Salvage Surgery: Saving More Than Just a Limb

One of the most promising advances in sarcoma treatment is limb salvage surgery, a procedure that removes the tumour while preserving the function of the affected limb. It’s a transformative option for patients who once faced amputation as their only path forward.

“Limb salvage helps patients maintain mobility and independence,” says Dr. Choong. “It also has a tremendous impact on emotional well-being.”

Thanks to advances in imaging, surgical techniques, and preoperative planning, limb salvage is increasingly successful particularly when sarcomas are caught early. In some cases, preoperative chemotherapy or radiotherapy can shrink the tumour, making surgery even more effective and less invasive.

Awareness and vigilance are key in the fight against sarcoma. Dr. Choong urges the public to take painless swelling seriously:

“If you notice a new lump or swelling that does not go away, do not wait. Talk to a doctor. Early detection can save your life and your quality of life.”
